Responsibility matrix
A responsibility matrix makes the boundaries between parties explicit. This guide explains how to read one and how it changes.
Roles
- Accountable: owns the outcome
- Performing: does the work
- Approving: must agree before the work proceeds
- Informed: is told about the result
Illustrative example
Illustrative only, not an authorization policy: for an observation-only asset check, the assigned qualified provider performs the scoped task, the authorized customer requester approves the request, and the applicable site work authority supplies any required site approval. The designated customer contact receives the observation or exception record. Actual roles follow the service agreement and site requirements.
A coordinator routes the request, tracks approvals, and preserves the record. Coordination is not permission to perform technical work or approve a customer configuration change. If the asset, instructions, or conditions differ from the authorized scope, pause and seek clarification through the agreed channel.
Interfaces and change control
The most important rows are interfaces, where one party's work affects another's. Changes to the matrix follow the change process and are versioned.
